latex undertilde.ins" in the terminal in this document folder to generate undertilde.sty.You can post a complete LyX file as attachment here, which produces an error. And even better, you can export to LaTeX (in the LyX main menu) and post the exported LaTeX file as attachment. Many, like me, can work with that, without having LyX installed on each machine.
