Page Layout ⇒ Two Column Layout and Two Column Thank You note

Hello,
I am preparing a document for a conference and using IEEEtran class.
The document is two column format and conference requires a thanks note which is in both columns.
Please see the attached file for more clarity.
It will be great if someone can help me format this easily.

Perhaps there was no answer yet because there's nothing to start yet. Would you like to post the code you already have, as a starting point, that can be improved? (A compilable copy without personal data or content)
