Hi,
I'm trying to use Latex for a court filing and am having problems with the great template posted by lawlist. The numbering is on the left edge and begins about halfway down the page. I don't know how to fix it; can someone please point me toward a solution? (I'm using Texmaker 4.5 on Fedora 23.

Hi,
Thanks for your prompt response. I am using that post as a source with no modification. I didn't accurately describe the problem I'm having so I'm attaching a pdf generated from that document. (I had found another sample which had similar issues to what I described.)
The code in the post is much more complex than what I've worked with in the past and I'd like to learn as the problem is being addressed. I have no idea where to begin analyzing the code.

Pleading paper layout
I have been using lawlist's code in lyx for years (now on lyx 2.2.2) and, with a little tinkering, it works fine. However, adding an index entry in lyx blows it up.
