The MISC document type did not work. Instead, I have enclosed "U.S. Department of Housing and Urban Development" in brackets {} in the author field of the TECHREPORT document class. This keeps that string in the bibliography exactly as typed, and the citation in the text is correct as well ...

This problem has been solved, with the help of Jelmer Ypma. His page contains a script he wrote to integrate Notepad++ with LaTeX and BibTeX:

Getting rid of \vspace and the rest worked perfectly. I don't need to use those other packages you mentioned for this project, although I appreciate the suggestions. Simply putting

\chapter{Chapter Title}
\newpage

as the beginning of each chapter file formats it just fine. Thank you ...
